What is Telehealth? PDF Print E-mail

Telehealth is the provision of health services at a distance using a range of technologies. Examples of telehealth include telephone or video consultations to support diagnosis and management, clinical networks and health professional education.

 

An example of telehealth in action is the remote electronic exchange of personal health data from a patient at home to medical staff at hospital or general practice to assist in diagnosis and monitoring. The main benefits are that clinical staff can monitor long-term conditions such as high blood pressure, diabetes and chronic obstructive pulmonary disease without the need for the patient to attend the surgery. The patient is taught to take their own regular health readings and feed them back to the surgery.

Telehealth in Moray – Digital Dufftown PDF Print E-mail
In Moray, the use of Telehealth is currently being piloted in Dufftown for patients with high blood pressure.

A group of 40 to 60 year old patients with established hypertension are taking part in a three phase project to see if their blood pressure can be controlled as well by telehealth monitoring as traditional care.

Phase 1 involved compiling a retrospective record of their last six months of traditional care readings.

Phase 2 involved teaching the patients how to use the electronic blood pressure machines. The machines were then handed in to the Health Centre at agreed intervals for the doctor to download, review results and feedback written instructions which were returned to the patient when they collected their machine.

Phase 3 involves the patient uploading their data to a secure website which is checked daily by the Practice, the results assessed by the doctor and feedback given by electronic message to the website within five working days which the patient then knows to check.

At the end of the project average blood pressures for the three phases will then be compared and patient feedback forms completed to evaluate the overall effects of the project.

Met Office Healthy Outlook® COPD Forecast Alert Service

 

Moray Community Health and Social Care Partnership has signed up to use the Met Office Healthy Outlook® COPD Forecast Alert service this coming Winter. The service will run until the end of March 2010.

We would like to invite any person in the Moray area who has been diagnosed with Chronic Obstructive Pulmonary Disease (“COPD”) to take advantage of this free service which aims is to help you to manage your condition and stay well throughout the Winter months.

 

The weather can affect the breathing of people with Chronic Obstructive Pulmonary Disease and can be made worse when there are viruses around that cause coughs and colds.  Healthy Outlook® is a preventative service that helps COPD patients manage their condition more effectively by warning them of periods of increased risk to their health.

 

If you decide to take part you will receive an automated telephone call, at a time suitable to you, when poor conditions are forecast.  During the call the expected conditions will be explained and the system will run through a few questions to ensure you are doing all that you can to help prevent your condition from becoming worse.

The calls will not happen every week.  They will only take place when the forecast indicates the weather could effect your condition.  When calls are made they will take place on a Tuesday or Wednesday and you can tell us what time you would prefer to be called on the slip below.

 

Last winter the service was used by over 8,000 people with COPD across the UK and eight out of ten people surveyed found the calls useful. If you do use the service this winter we would like to know your thoughts by asking you to complete a short questionnaire in April 2010.
